This II No Consensus Lisbon Spine Meeting 2026 has been dedicated to the new trend of surgical techniques to treat Adolescent Idiopathic Scoliosis (AIS) that fail conservative treatment.
Over the last couple of decades paediatric spine deformity surgeons have been searching for new techniques that would overcome the drawbacks of spinal fusion in AIS. We all know that by fusing a segment of the spine it would increase the strain on the adjacent levels that may lead to degenerative discopathies years later and very often, if the post-op spinal alignment is not perfect, it will increase the chances of developing backpain later in life. Also fusing the spine in the early adolescent growth spurt will stop the growth of that segment of the vertebral column with all its disadvantages and consequences. So over the last decade new techniques that would aim to preserve spinal motion and growth on one hand, but would at the same time correct the 3D AIS spinal deformity have been developed. Doing it from the front or from the back, instrumenting the spine either with screws and tethers, dynamic spring devices or removable screws and rods, all these alternatives are on the market but follow up is short and basically none of them have proved the test of time.
Indications for surgery in each of these techniques are not clear cut and therefore need to be somehow clarified. Are they the new generation of procedures we have been waiting for decades or are they just another tool in our surgical armamentarium ? Are they better than a shorter fusion (selected fusion) already with mid and long-term follow ups? Some of these procedures are extremely aggressive for our adolescent patients with spinal deformities but are they worth it? Are all our AIS patients candidates for these techniques?
This is the first meeting in the western world fully dedicated to Spinal Tethering and Growth Modulation in AIS and we do hope to bring some consensus in certain areas. We have gathered a group of world experts on the different techniques and I am sure you will enjoy the discussion and learn from the interaction between all of them.
